I still use 'medieval' to refer to the roughly 1000-year period before the onset of the Columbian exchange in Afro-Eurasia, and it would be pretty handy if 'premodern' could mean Antiquity + the Middle Ages in that sense (or the pre-Columbian period in the Americas). It isn't typically used that way, though, which I suppose is my main problem with the term.
